your selection is right. the “Beitragsnummer” is a 9 digit number provided by the person already paying.
second screenshot you just add your “Aktenzeichen” and Zip code which is provided in the letter YOU got, so the system can connect your submitted form immidiatly to your the correct folder.

I really nicht möchte stören and I’m nicht a big help, but I just möchte sagen that I love this gebrauch of Denglish. (German and English at the same time)
