The same patient can exist as an EHR record at a hospital, a claim at a health plan, and a paper file at a community provider. Effie Carlson, CEO of Watershed Health, says the information may all be there while the people responsible for the patient still struggle to manage what happens next.
Watershed connects hospitals, health plans, physician groups, post-acute providers, transportation companies, and community organizations without requiring them to replace the tools they already use. Its platform brings together information from separate systems and turns it into shared workflows for referrals, care transitions, and gaps in care. The company’s network now includes nearly 12 million patients across 14 states.
Effie has worked across physician practices, health plans, home health, government relations, and healthcare technology. In each part of the industry, she saw plans unravel when responsibility passed between organizations. Her experience also shaped Watershed’s view that no single platform can account for the many ways healthcare providers work.
