it’s not because up till now abortion is still considered killing and professionals are duty bound to report criminal cases.
Even a priest or a psychologist is bound by professional secrecy, but if you confess about murdering someone or discuss it with your therapist, they are bound to report it.

Depends, laws can say that when reporting possible crimes it takes priority over the privilege.
