South Africa was alerted to the employee’s behavior more than once and said it didn’t rise to the level of removing that employee. That’s apparently because the employee agreed to more psyche assessments and writing an apology letter where they expressed remorse. That seems less than comforting.
Add to that, that psychologists aren’t across the board required to be full time staff even after multiple incidents of violence and it seems like this is going to keep happening. Space bases would probably need a central governing body to ensure safety.
