There is a simple solution to this: deposit 100k on an independent bank account before doing that BS and agree that you will be left to die once that money was used up.
Or, simply, agree not to want any treatment if you can’t afford it.
I do not understand why we would not be able to bill the country of origin for the accidents that were not paid in full by the person that was involved.

When a visiting family member had to go to the hospital in NW many years ago, they wouldn’t touch her before I made a deposit to the hospital, and she had a good travel insurance, and they called the hospital before she went there – I tought this was common practise.
