>A staff member who was supposed to be looking after her has now admitted to falsifying these records.
Then he must be prosecuted for gross negligence manslaughter. Simple as that.

I certainly hope those two members of staff were fired and/or forced to undergo extensive retraining. Ideally the former, and never allowed to work in hospitals or anything care-related again.

So to translate, they’d decided she had a personality disorder, wasn’t “really mentally ill”, and was attention seeking, had never had any intent to kill herself and were angry that she was causing them extra work.
RIP Cerys and all the other thousands of young women who have been, and are being, let down by the NHS. Funding may be pitiful, staff morale low, wards dangerously understaffed but the attitude of a good percentage of workers is a disgrace and kills.

It’s horrendously sad to see another life slipping through the cracks of our care services … I wouldn’t be surprised if the falsifying of records was a standard practice.
People in jobs either poorly trained or supervised or told just to do.
This example is one of many happening on a daily basis, overloaded people making bad decisions that have serious consequences.
Maybe she would have gone to have a better life eventually when she reconciled the demons, maybe she never will.
But now she is at rest .. while those left behind can only grieve … night night poppet

Not to sound morbid, but I’ve been on these wards (as a staff member), and most toilets are anti-ligature.
How was she even able to kill herself in a mental health wards toilets?

My partner has had issues that involved our local MH team. About 25% of the team are brilliant, 50% ok and 25% bloody awful and incompetent. First time she was in for 2 weeks observation and assessment. I turned up to visit. Asked at the desk how has she been? The young doctor got her notes and said she has spent most of the day on her phone ! At that point I took her phone out of my pocket that I had taken the previous day for charging and asked did they know who’s phone she had borrowed?
On another occasion I was sat on a ward a 4 male nurses tried forcing a patient to take their medication. I knew from previous visits they had self admitted and taking medication was voluntary. They started to strong arm the individual. I stood up and said I would arrest them all for assault if they continued. (I am not a policeman but am always mistaken for one). They left to their rest room immediately.
Unfortunately mental health isn’t glamorous and struggles for funding. Especially when at least 1 in 4 people will need assistance at some point in their life.
Let’s also not forget that the largest amounts of money paid out of the NHS budget are for the public / private sector hospitals that Gordon Brown organised at Credit Card interest rates.
