This is not a pet passport. It’s a vaccination card with some space for treatment description.
It’s an old entry regarding treatment of ear infections, possible with something else (skin? Urinary?).
The 1st one is an injectable antibiotic, commonly used in Poland for severe ear infections or common urinary tract infections, respiratory or skin infections.
The 3rd one is a popular injectable steroid, mainly used to control severe inflammation / itchiness.
The last one is a common antibiotic ear drops used for treatment of external ear infection.
Anyway, it’s not any official document and can be discarded if your dog is doing all good and have no any health concerns.

My guess is: Enrobioflox 3.0, most likely Sultrim 3.0, Dexafort 1.0, Catosal 1.0, and Suro-something? Unsure about the last one could possibly be Surolan or surosolve
