Tbilisi State University and TSU as short in the middle, you can see the date 1918-1968
It says “Tbilisi State University” in Georgian. My guess is that it’s a medal for the 50th anniversary of the university.

It says:
თბილისის სახელმწიფო უნივერსიტეტი = Tbilisi State University
თსუ = TSU, abbreviation for the university name
თბილისის სახელმწიფო უნივერსიტეტი – Tbilisi State University
