    Is the system perfect? No. Do mistakes happen? Absolutely yes. Is the NHS still better than dying from the lack of insurance? Also yes.

    I don’t understand how this happened. When I had surgery, they checked my name, address and surgery type about 20 times. Even when I hadn’t moved and it was just someone new talking to me. I confirmed my details at least three times while in the theatre too, with the same people in the room. So I can’t understand how this could happen unless every single person in the chain is just skipping procedure.

    What most people don’t realise is that 5% of all death in NHS hospitals are preventable.
    According to different sources, about 1,500 deaths in hospitals in 2022 were preventable. And by preventable , I mean that those deaths were directly related to medical negligence and mistakes.

  5. Not really “revealed”, never events are routinely published.

    The most common one is wrong site surgery and amongst that it’s wrong tooth removed, normally in a mouth full of similarly manky teeth.

    They shouldn’t happen (hence never event), but we record them and hopefully learn from the tiny amount that do happen to make systems safer.
