I think its meant to show that the back thingy opens so a person can come out.
Answering your question, i dont think pulling someone in a whellchair behind your car is legal because safety on the road or something like that

Not only is it legal but it’s encouraged. The police over here are known for pulling people over if they see a disabled badge and ordering them to pull the chair on a rope.

Perfectly legal here in Poland, we call it „kulig” and some people get to do it even as adults if they move on a wheelchair
