This bloke almost certainly tows something heavy. The extra wheels are for load capacity on the rear end. Don’t suppose it had a fifth wheeler attached inside the tray ?

Plenty of unnecessary yank tanks but this one might have a purpose: towing large trailers or floats. Unable to tell from the photo but they sometimes have a Fifth Wheel hitch mounted in the tray. It would explain the dual rear wheel axle.
