Applying for Chancenkarte for job search in Berlin – online form not working

Hi everyone,

I’m currently in Berlin and trying to apply for the Chancenkarte (opportunity card) for job search via the official service page:

However, when I start the online application form, it opens up the Temporary residence permit for employment application form.

I would like to know

1. If anyone here successfully applied for the Chancenkarte in Berlin in last few months?
2. How did you submit your application (online, email, in person)?If in person or email please let me know how to get in touch with them.
3. Is there a way to overcome this bug?

I have read that contacting LEA via application form doesnt work as they dont get back to our messages. Please let me know what to do

Any advice or personal experience would be really helpful! Please guys I really need some help as time is running out for me, nothing seems to work out and I am too stressed. Any help or idea is welcome. Please let me know.
